Dear Parents:

For your child to be registered in our Faith Formation classes, the following forms and documents must be in place. Failure to provide the required documentation will result in the postponement of receiving the sacrament.

**All parents must sign a copy of the Circle of Grace when they bring their child to their first class.

First Communion Documents

Students wanting to make their First Communion must present the following documents:

1. Class Registration Form
- Copy of Birth Certificate (official document)
- Copy of Baptismal Certificate (official document) **Please note that the child must be seven years old by September 15 in order to register for First Holy Communion Classes. Also, sponsors (padrinos) are not required for students making their First Communion.
